Liability Disclaimer

I, the undersigned parent or legal guardian, hereby give permission for my child to participate in the church-sponsored youth event to Anchor Bend Church on Wednesday March 26th. I understand that while every reasonable effort will be made to ensure the safety of all participants, Space City Church, its staff, volunteers, and representatives shall not be held liable for any injury, accident, illness, loss, or damage that may occur during, traveling to/from, or as a result of participation in this event.

I acknowledge and accept all risks associated with this activity and agree to release and hold harmless Space City Church, its pastors, leaders, employees, volunteers, and agents from any and all liability or claims arising from participation in this trip.

In the event of an emergency, I authorize the adult leaders to seek and obtain medical treatment for my child if necessary, understanding that every effort will be made to contact me prior to any such treatment.
